What is Ultherapy ? How does it work ?
Ulthera is a non-invasive facial tightening device that uses heat energy to stimulate collagen in the skin layers. It helps tighten and firm sagging facial and neck skin, restoring a more youthful appearance. Whether it’s lifting the eyebrows, lifting droopy eyelids, lifting sagging cheeks, reducing cheek lines, or tightening the jawline.
How does Ultherapy work ?
Ulthera is a non-surgical skin tightening technology that utilizes high-frequency ultrasound energy to penetrate the skin layers. The ultrasound waves are then converted into heat energy, reaching temperatures of 65-70°C, which accumulates in the skin and stimulates collagen production, causing the skin to contract and tighten.
The primary target of this energy delivery is the SMAS layer, which supports the entire facial structure. If the SMAS layer becomes lax, it can lead to changes in facial structure such as droopy eyelids, sagging cheeks, and jowls. By tightening the SMAS layer, the facial structure is restored, reducing sagging, defining the jawline, and providing a more youthful appearance.
Ultherapy procedure
After applying the numbing cream and it has taken effect, a gel will be applied to act as a medium for directing energy into the skin layers. Then, the doctor will use the Ulthera device to scan and determine the depth of the SMAS (Superficial Muscular Aponeurotic System) layer in the patient’s skin. Since the depth of the SMAS layer varies among individuals due to factors like facial fat and skin thickness, the doctor will select the appropriate depth setting on the Ulthera device to match the SMAS layer’s depth in the patient’s face. Following this, the doctor will start emitting energy into the skin layer.
The importance of Ultherapy Real-Time monitor
The monitor screen is crucial because it helps the doctor see how deep the SMAS layer of each patient is. This allows the doctor to select the depth level of the device’s applicator to be as close as possible to the depth of the patient’s skin layer. This ensures both the doctor and the patient that every shot of energy delivered will accurately target the SMAS layer.
Does the result depends on depth of the energy ?
Yes. However, the deeper level the energy reaches does not mean the better result. Human skin consist of many layers (from shallow to deep respectively):
- Skin layer (Dermis)
- Fat Tissue layer (Subcutaneous Fat)
- Muscle layer (SMAS layer);The energy need to target precisely at this layer to give result.
- Bone
When the energy reaches too shallow: your skin will not get lifting effect.
- Cumulation of heat in Dermis layer will result in pain, skin burns and blister.
- Cumulation of heat in Fat tissue layer will not hurt and may not have serious complication. However, the only thing is that it won’t give you the lifting effect.
When the energy reaches too deep: The energy is absorbed in Bone layer instead of SMAS layer, which can be really painful and won’t give you any lifting effect. Some patient cannot tolerate the pain and might not want to do Ultherapy anymore due to bad experience.
How many levels of depth does the Ultherapy applicator have?
The answer is 3 levels of depth:
• 1.5 millimeters
• 3.0 millimeters
• 4.5 millimeters
The reason why the Ultherapy device needs multiple depth levels applicator is that the SMAS skin layer depth varies for each patient’s face. Even within the same face, the SMAS layer depth on the forehead and cheeks may differ. To ensure that every shot of energy targets the SMAS skin layer in each area as accurately as possible, there must be multiple depth levels.
How long does Ultherapy takes ?
- After finishing registration and taking ‘before photo’
- Then applying topical numbing cream for 1-2 hours to ensure maximum comfort while doing the treatment.
- Next is to start Ultherapy procedure which could take approximately 1-2 hour depends on how many areas to be treat.
- Overall, it would take around 3-4 hours in total for 1 Ultherapy session.
How painful is Ultherapy?
During Ulthera treatment, you can feel the heat energy penetrating into the skin. There might be a slight sensation of discomfort and numbness. However, with the doctor’s technique combined with the application of numbing cream, most patients feel minimal pain. It’s a tolerable discomfort considering the results achieved.

What to expect after Ultherapy ?
You can see immediate results of about 30% because after stimulating collagen with thermal energy, it requires time for the collagen to contract and tighten. Full results of 100% are seen after two months.
Does Ultherapy actually work?

Swelling is common especially with patient who have sensitive skin. Swelling will last 1-2 weeks after the procedure.
How to prevent swelling ?
You can divide treatment into two session instead of one session. For example, if you decided to do 800 lines, you can do 500 lines in the first session and 300 lines the next session but both session should be within 1 months. Dividing into two session will still give the same result as doing 800 lines in one session, the advantage is there will be less swelling.
The difference between HIFU and Ultherapy
Both HIFU and Ultherapy help tighten your skin. Their energy aim at same layer which is SMAS layer. The differences are the imaging technology, level of pain and price of treatment.
Who should not do Ultherapy ?
- Patient who have metal implant should avoid doing Ultherapy. Eg. Patient who have done facial bone surgery.
- Patient who have nerve problem or Myofascial Pain Syndrome because it may cause undesired complication.
- Patient who have heart device implant; pacemaker.
How to prepare for Ultherapy treatment
There is no specific thing to prepare, except time because it will take approximately 3-4 hours to do one session.
Ultherapy after care guideline
Patient should not take pain-control pills, anti-inflammatory pills or immunosuppressive pills. Taking these medicine will reduce the lifting effect.
